Using your travel insurance for medical care in Costa Rica

Insurance only helps if the hospital, the insurer and the doctor talk to each other in real time. This is how that works in Costa Rica — and how to avoid paying the bill twice.

  • Call your insurer's assistance line (on the back of your card) as early as possible; they open a case number and can authorize care.
  • Private hospitals in Costa Rica accept a guarantee of payment (GOP) from international insurers — direct billing instead of reimbursement.
  • A physician-led coordinator gets the GOP opened faster, because the medical estimate and the reports come from the treating doctor.

Assistance company, insurer and hospital: who does what

Your policy is issued by an insurer, but the phone number on your card usually reaches an assistance company that operates 24/7 on the insurer's behalf. It opens a case, verifies coverage, and issues the guarantee of payment (GOP) to the hospital. The hospital treats and bills; the insurer pays. In Costa Rica's private hospitals this circuit is routine — with Relevium it is also fast, because the medical estimate and the reports the assistance company needs come straight from the treating physician.

Direct billing versus reimbursement

With a GOP, the hospital bills the insurer directly and you pay only your deductible or copay. Without one, you pay the hospital and claim reimbursement later — which requires itemized invoices, medical reports and often translation. Many visitors end up paying out of pocket simply because nobody opened the GOP in time. Message us before you pay: opening the guarantee is the first thing we do.

What your insurer needs from the doctor

A first medical report with diagnosis, planned treatment and estimated cost; progress reports if the stay is longer than a day or two; a discharge report with medication and follow-up; and, for the trip home, a fit-to-fly assessment. Health insurance for visitors and expats

Short-term visitors normally rely on travel insurance with medical coverage and repatriation. Longer-term visitors and expats use international health plans or local private insurance; residents are also enrolled in the public CCSS system.
