An English-speaking doctor in Costa Rica, 24/7

Being sick abroad is harder when you cannot explain what is wrong. Where English is easy to find in Costa Rica, where it is not, and how to reach a physician who speaks it at any hour — and stays on your case until you fly home.

  • Relevium's WhatsApp line is answered by a physician 24 hours a day, every day — in English, Spanish or Portuguese.
  • English is common in the private hospitals of San José; in the public system and in small beach-town clinics it is not guaranteed.
  • In a life-threatening emergency call 911 first, then message us so a physician can coordinate the next step.

Where English is easy — and where it is not

The large private hospitals of the San José area — Clínica Bíblica, La Católica, CIMA, Metropolitano — see international patients every day: English is common among physicians and administrative staff, and several publish their websites in English. That is where the language barrier is lowest.

It rises fast outside them. In the public system (the CCSS, "la Caja") English depends on who is on shift. Small private clinics in beach and mountain towns are good at what they do, but their English varies, especially at night. Pharmacies, laboratories and ambulance crews may switch to Spanish when things get technical — and that is exactly when a misunderstanding about a dose or an allergy matters most.

What happens when you message Relevium

A specialist in Emergency Medicine reads your message and replies — in English — with the questions that matter: what happened, since when, what you take, what you are allergic to and where you are. From there the physician decides the level of care your case needs.

  • Advice and a plan by message, when that is all it takes.
  • A consultation, with the right prescription and instructions you understand.
  • Admission to a private hospital, coordinated before you arrive, with specialist assessment usually available in under six hours.
  • Transfer to San José when the local option is not enough.

One physician, one language, the whole way

The physician who answers stays on as your physician of record: they speak to the specialists, the hospital and your insurer, and explain every decision to you and your family in plain English. Daily updates go out by WhatsApp or phone, so relatives at home are not left translating hospital Spanish.

Reports and paperwork in English

Insurers and doctors back home need documents they can read. Relevium issues structured medical reports in English at each milestone — admission, progress, discharge and, if you fly home, the fit-to-fly assessment — and consolidates the hospital's separate invoices into one with CPT codes. Spanish and Portuguese versions are available on request.

What to send in your first message

The faster we have the essentials, the faster a physician can act — and, if you are insured, the sooner the guarantee of payment can be opened.

  • Where you are: hotel or address, town and a landmark.
  • What happened, when it started and how it has changed.
  • Age, regular medication and allergies (a photo of the box is fine).
  • A photo of your insurance card and the assistance-line number, if you have one.
  • A phone number where we can call you.

If you work with tourists

Hotel receptionists, guides and tour operators are often the first to know a guest is unwell. Save the number: you can message us yourself with the guest's details, and a physician continues the conversation with them in their language.

When to call 911 first

Some situations cannot wait for a message: someone unconscious or not breathing, chest pain with shortness of breath, signs of a stroke (face drooping, arm weakness, slurred speech), heavy bleeding or a serious head injury. Call 911 — free from any phone, nationwide — and message us once help is on the way, so we can take the case from the ambulance onward.

This guide is general information, not medical advice for a specific person. In an emergency call 911 or message our 24/7 line.

English-speaking doctor — frequently asked questions

Many do, especially in the private hospitals of San José. It is less predictable in the public system and in small clinics outside the capital. Relevium handles every case in English from the first message to the final report.
