Urgent care across Costa Rica: region by region
Costa Rica's best hospitals are in San José; its best beaches are hours away. What exists locally, what does not, and how a physician decides when to move a patient.
- Every tourist region has private clinics for consultations, wounds, IV fluids and basic imaging; ICU-level care and most surgery are in San José.
- Road time to San José: 1.5–2 h from Jacó, 3–4 h from Liberia, 4–5 h from Tamarindo or Nosara, about 4.5 h from Puerto Viejo.

Guanacaste: Liberia, Tamarindo, Flamingo, Nosara, Sámara
Liberia has an international airport (LIR) and a private hospital with emergency, imaging and surgery — the regional referral point. Tamarindo, Flamingo and Nosara have private clinics handling consultations, minor trauma, IV rehydration and stabilization; several coordinate ground ambulances to Liberia or San José. For serious trauma, cardiac events or anything needing ICU, the destination is San José: 3.5–4 hours by road from Liberia, longer from the beaches, or under an hour by air ambulance from LIR.
Central Pacific: Jacó, Quepos and Manuel Antonio, Uvita
Jacó is the closest beach to the capital — about 1.5–2 hours — with private clinics and a public clinic; Quepos has a public hospital and private clinics; Uvita and the Costa Ballena rely on smaller private practices. Surf injuries, crocodile-river and wildlife incidents, and traffic accidents on Route 34 are the typical calls. From this coast a ground transfer to San José is usually the right choice; Relevium notifies the receiving hospital so the patient goes straight to the team.
Caribbean: Puerto Viejo, Cahuita, Limón
Limón has a public regional hospital; Puerto Viejo and Cahuita have small clinics. The road to San José is 4–5 hours and sometimes closed by landslides in the rainy season. Early decisions matter more here: a physician assessing by message and photo within minutes can spare a patient a long, unnecessary trip — or start one before the road is the problem.
Arenal, La Fortuna and Monteverde
La Fortuna has private clinics and a public clinic; Monteverde's care is basic and the road out is slow. Both are about three hours from San José. Volcano hikes, hanging bridges and canyoning produce sprains, fractures and the occasional cardiac event in visitors who overestimate the altitude and the humidity.
How the transfer decision is made
Not every case should move: a stable fracture, a stomach infection or a dengue case without warning signs is often better treated locally. A head injury, chest pain, a surgical abdomen, or any patient whose condition may deteriorate belongs in San José — by ground ambulance when the drive is short and the patient stable, by helicopter or fixed-wing when time or terrain says otherwise.
